J.P. SMALL FOUNDATION
SCHOLARSHIP PROGRAM

 The J.P. Small foundation Scholarship Program provides comprehensive support services to students enrolled at institutions of higher education:

        . Four year scholarship awards of up to $1,500.00 annually.

       . Personal and career counseling available on a year-round basis.

       . An annual weekend of interaction with other J.P. Small scholars from around the first Coast.

 ELIGIBILITY REQUIREMENTS

•  A high school male and female senior accepted by a four year accredited college, university, vocational or technology.

•  High academic GPA of 2.5

•  SAT 900 or ACT 23 minimum scores.

•  Leadership potential

•  Financial need

 A. INSTRUCTIONS FOR SUBMITTING THE APPLICATION

 The application must be typed. All pages of the application must be numbered at the bottom. (Applications that are hand written and /or submitted without ALL required documentation will be Disqualified.) There are to be no hand written documents!!!!!!

 Required Documentation (must accompany the application)

 1. Official high school transcript

2. Official score reports for the Scholastic Aptitude Test (SAT) or the American College Test (ACT) (copies accepted)

3. School certification form (5)

4. Personal essay

5. Letter of recommendation from your guidance counselor

 FAXED applications or required documentations WILL NOT BE ACCEPTED
